HETIRISK has experience designing and conducting a variety of risk improvement training programs - emphasizing human error, behavioral-based intervention, incident investigation and root cause analysis, and process improvement - that enhance operational results.
Representative Projects*
Construction Firm - Conducted two-day incident investigation and root cause training workshops for supervisors of a construction company that included using structured analytical techniques to determine root causes and contributing factors. Such techniques included events and causal factors charting, change analysis, hazard-barrier-target analysis, cause-and-effect diagram, tree diagramming, and behavior analysis. Identifying sources of evidence and mapping/diagramming techniques were also part of the training.
Building Materials Distribution - Designed and wrote an eight-hour defensive driving program and presented to national building materials distribution firm with heavy fleets in 10 locations throughout the Western US and more than 500 drivers.
Solid Waste Facilities - Trained and assisted a solid waste facilities management and operations team in developing and implementing solid waste facility risk management systems to protect people, property and the environment. Root cause analysis and incident investigation procedures were implemented. Efforts contributed to loss ratio of less than 30% and no significant claims in the following four years.
